Understanding Hormonal Hair Loss (Androgenetic Alopecia)

Hair loss related to hormonal imbalances is common and can be caused by various factors. Here’s a comprehensive overview to help you understand the primary causes, medical conditions involved, and other influencing factors. 

Main Causes of Hormonal Hair Loss

1. Dihydrotestosterone (DHT)

A derivative of testosterone, DHT binds to hair follicles, causing them to shrink. This shortens the hair growth cycle and eventually leads to hair thinning or cessation of hair production. 

2. Androgenetic Alopecia (Pattern Baldness)

Often hereditary, this condition causes hair follicles to become more sensitive to androgens like DHT, resulting in progressive hair thinning in genetically predisposed individuals. 

3. Hormonal Fluctuations

Hormonal shifts can disrupt the hair growth cycle:

  • Pregnancy: Elevated estrogen prolongs the growth phase, but postpartum drops can cause hair shedding.
  • Menopause: Reduced estrogen and progesterone levels contribute to thinning hair.
  • Other shifts: Stress, illness, or hormonal therapies can influence hair health.

Medical Conditions Affecting Hair Loss

4. Endocrine Disorders

Conditions such as Polycystic Ovary Syndrome (PCOS) and thyroid issues (hypothyroidism or hyperthyroidism) cause hormonal imbalances that can lead to hair thinning. 

5. Elevated Insulin Levels

Particularly in PCOS, high insulin can increase androgen activity, contributing to hair loss. 

Other Factors Influencing Hair Health

6. Stress and Cortisol

Elevated cortisol levels from stress can induce a shedding phase called telogen effluvium. 

7. Genetics

A family history of hair loss increases susceptibility due to inherited sensitivity of hair follicles to androgens. 

Impact of GLP-1 Receptor Agonists on Hair Loss

Recent observations have indicated that some patients using GLP-1 receptor agonists (medications used for diabetes, weight loss, and metabolic health) may experience hair shedding or thinning. The exact mechanism is still under study, but it is thought to involve hormonal modulation and changes in metabolic pathways affecting hair follicle cycling.
